Job Description

The Outpatient Physical Therapist (PT) evaluates all assigned patients, developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. The PT is responsible for delegating and directing all discipline-specific therapy services for assigned patients and ensuring the design of functional treatment programs for effective return to community activities. The role includes serving as an educator for patients, families, students, and coworkers while performing professional duties related to patient care and the development of therapy services and program delivery.

Responsibilities

• Complete comprehensive evaluations and develop appropriate treatment plans focusing on patient progression towards discharge.
• Document medical records clearly, concisely, and accurately in a timely manner, ensuring compliance with all guidelines.
• Provide mentorship and supervision for students in the therapy program.
• Actively engage in professional development and pursue continuing education opportunities.
• Adhere to the State Practice Act and maintain professional licensure in good standing.
• Participate in quality improvement initiatives within the organization.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Active CPR certification.
• Must hold and maintain an active and applicable license for the state where work will be performed.
