Job Title: LPN – Night Shift
Location: Flagstaff, AZ
Type: Contract
Shift & Schedule: Nights | 6:00 PM – 6:00 AM | 4 Days/Week | Up to 40 Hours/Week | Rotating Block Schedule | 2 Consecutive Days Off | Every Other Weekend
Pay Rate: $40 - $44/hr (Based on Experience)

Position Overview:
We are seeking a dedicated LPN to provide direct nursing care to residents, including medication administration, treatments, assessments, documentation, and coordination of care.

Responsibilities

• Administer medications and perform treatments as ordered.
• Provide wound care and dressing changes.
• Perform tracheostomy, gastrostomy, jejunostomy, and catheter care.
• Monitor residents for changes in condition and respond to emergencies.
• Complete nursing assessments and maintain accurate documentation.
• Assist with MDS assessments, quarterly reviews, and individualized care plans.
• Communicate with residents, families, physicians, and interdisciplinary teams.
• Participate in QAPI programs and staff meetings.
• Follow nursing regulations, facility policies, and medication-control procedures.

Required Qualifications

• Active LPN/LVN license eligible for practice in Arizona.
• Knowledge of geriatric nursing and patient-care practices.
• Knowledge of pharmacology, medication administration, and treatment plans.
• Strong observation, assessment, and clinical judgment skills.
• Ability to recognize and respond appropriately to medical emergencies.
• Strong communication, documentation, and teamwork skills.
• Ability to work a rotating block schedule and every other weekend.
