Social Worker - Bridge Clinic (Addiction Care)

The General Hospital Corporation is seeking a Social Worker for the Bridge Clinic in Boston. You will provide psychosocial assessment, treatment planning, and brief CBT, motivational interviewing, and psychoeducation to patients with co-occurring mental health and substance use disorders. You' ll coordinate care with a multidisciplinary team, perform rapid assessment and crisis management, and support transition planning for patients across addiction treatment and mental health services within #J-18808-Ljbffr Massachusetts General Hospital
