A strong first impression.We are seeking a full-time Hospice Admissions Nurse to complete hospice admissions and support patients and families during the transition into hospice care. This role serves hospice patients receiving services in their own homes or facility settings. Normal operating hours are Monday through Friday, 8:30 AM to 4:30 PM.What We OfferMedical, dental, and vision coverage401(k) with employer matchHealth Savings Account eligibility with employer contributionCompany-paid Life and AD&DVoluntary disability, accident, critical illness, and hospital indemnity optionsEmployee Assistance Program and supportive resourcesTuition reimbursementPaid time off and paid holidaysMileage reimbursement per company policyPosition SummaryCompletes hospice admissions by assessing eligibility, educating patients and families, and facilitating a smooth transition into hospice services.Serves as a key clinical liaison between referral sources, patients, families, and the interdisciplinary team.Key ResponsibilitiesConduct hospice admissions in private homes or facility settings.Assess patient condition, hospice eligibility, and appropriateness for services.Educate patients and families on hospice philosophy, services, and expectations.Obtain required consents and complete admission documentation accurately and timely.Collaborate with referral sources, physicians, and internal team members to coordinate admissions.Communicate admission details and clinical findings to the interdisciplinary team.Document all admission activity in HCHB (Homecare Homebase).Travel within the assigned service area to complete admissions.Active RN licensure in the state of practice.One year of nursing experience required; hospice and/or admissions experience preferred.Strong communication skills and comfort leading sensitive conversations.Ability to work efficiently in time-sensitive situations.Reliable transportation for home and facility visits.EMR experience preferred; HCHB familiarity a plus.Work ExpectationsField-based role serving hospice patients in home and facility environments.Manages admissions workflow with flexibility and responsiveness.Maintains timely documentation and clear communication with clinical leadership and referral partners.Work Location: In person #J-18808-Ljbffr The Care Team Hospice Care
