Weekend Evening Counselor

Position Weekend Evening Counselor at Concern Housing Location Riverhead, NY Days / Hours Saturday & Sunday; 4pm - 12am Overview Concern Housing is a non‑profit agency dedicated to providing housing and support services. We are one of the largest agencies of its kind in New York State, serving approximately 1,300 individuals and families across 240 locations. The Weekend Evening Counselor is responsible for assisting the Supervisor and Case Managers in delivering direct services to clients and in overseeing day‑to‑day operation of the site. Responsibilities Provides direct care services to a caseload of clients. Responsible for documentation of clients assigned to as well as documentation relating to other clients' progress. Monitors the site, coordinates and participates in house activities such as preparation of meals, chore activities, recreational activities, safety checks, etc. Facilitates food‑planning committee and trains clients in weekly food planning. Provides rehabilitative support services in daily living skills, health services and other assigned duties. Provides housing services including assistance with maintenance issues. Monitors medication. Assists with transportation of clients. Attends staff meetings as scheduled. Performs monthly fire inspections. Remains current with all agency‑required compliance documents and trainings. Requirements Requires a minimum of a high school diploma or equivalent along with at least two years of experience in a similar role.
