Primary Care Physician (Internal Medicine & Family Medicine)Location: Bakersfield Schedule: Monday - Friday (Day Shift) Setting: OutpatientJob Summary:We are seeking a dedicated and compassionate Primary Care Physician to provide comprehensive medical care to participants within an integrated healthcare setting. Under the supervision of the Medical Director and in coordination with the Program Director, you will be responsible for delivering high-quality, participant-centered care specifically tailored for frail and elderly populations.Roles & Responsibilities:Provide comprehensive primary care services, including assessments, diagnosis, treatment, and chronic disease management for adult and geriatric patients.Conduct physical examinations, review medical histories, order diagnostic tests, and develop individualized treatment plans.Collaborate with the interdisciplinary team (IDT) to coordinate and monitor participant care and overall health outcomes.Prescribe and manage medications, preventive care, and health promotion programs for patients.Educate patients and families on health maintenance, chronic conditions, and treatment plans.Supervise and collaborate with Nurse Practitioners and Physician Assistants while coordinating specialist referrals as needed.Maintain accurate medical documentation, participate in quality improvement initiatives, and ensure compliance with organizational policies and healthcare regulations.Qualifications & Experience:Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institutionActive California medical license requiredCurrent DEA registration requiredBoard Certified in Internal Medicine or Family Medicine IS REQUIREDMinimum of 1 year of experience working with frail or elderly populationsGeriatric care experience or added qualifications in geriatrics REQUIREDBenefits:401(k)Health, Dental, Vision & Life insuranceEmployee assistance programEmployee discountFlexible spending accountHealth savings accountPaid sick timePaid time offReferral programRetirement plan
