Job Overview Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Hilliard, Florida. Job Details Specialty: Physical Therapist Discipline: Therapy Start Date: June 15, 2026 Duration: 13 weeks Hours: 40 hours per week (8 hours per shift) Employment Type: Travel Responsibilities Help patients recover from injuries and manage chronic conditions. Provide therapy and rehabilitation services in a skilled nursing facility setting. Utilize restorative nursing programs, ADL training, and fall prevention strategies. Complete Medicare/Medicaid documentation and skilled notes. Work with geriatric patient population and SNF equipment such as Hoyer lifts and standard therapeutic modalities. Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Florida Physical Therapist license required to start the assignment. BLS Certification from AHA or ARC (may be required). Experience working in skilled nursing facility (SNF) settings preferred. Documentation of recent experience with restorative nursing programs, ADL training, and fall prevention preferred. Demonstrated ability to complete Medicare/Medicaid documentation and skilled notes preferred. Assignment Details Facility Type: Skilled Nursing Facility.
