Responsibilities
- Perform comprehensive PT evaluations for residents in the SNF setting and establish measurable treatment goals.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence.
- Provide therapeutic exercise programs, gait and transfer training, ROM techniques, and pain management as appropriate.
- Monitor patient progress, modify treatment plans as needed, and document outcomes accurately in accordance with policy and regulatory standards.
- Educate residents and families on exercises, safety, and discharge planning; coordinate care with the interdisciplinary team.

Requirements
- Active Physical Therapist license in the state of Washington or eligibility to practice in WA.
- Ability to work effectively in a skilled nursing facility setting with geriatric patients.
- Strong communication and documentation skills; professional and collaborative approach.
- Willingness to commit to a 13-week travel assignment; ability to travel as required.
- Prior SNF or geriatric PT experience is preferred.
