SUMMARY OF DUTIES Provides short-term outreach and home visiting services, as well as self-sufficiency education classes to at-risk families. Engages families in services. Conducts intake/assessment with participants and create service plan and goals. Teaches and provides information on a wide variety of topics, including Kauai community resources, budget and finance, economic stability and vocational development, as indicated by participant needs. Provides linkage to community resources and services such as counseling, legal aid, and vocational assistance. Assists families, through advocacy or concrete help, to negotiate systems to receive needed services. Provides transportation to medical, legal and as needed appointments with consultation with supervisor. Performs administrative tasks related to caseload such as progress notes, service plans, mileage and miscellaneous reports in a timely manner. Maintains participant confidentiality at all times. EDUCATION AND TRAINING REQUIREMENTS Four-Year College Degree from a school accredited by a recognized accrediting agency - general major. Requires basic knowledge or training in a specialized or technical field or trade, such as social work, professional accounting, finance, business administration, and marketing. EXPERIENCE Over two years, up to and including four years. Other (Specify): Experience working with families, who have abused or neglected their children, or high-risk families, preferred. Case management experience preferred. SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED Knowledge of child growth, development and behavior. Knowledge of positive parenting practices. Knowledge of case management and advocacy practices and techniques. Good written and verbal communication skills. Ability to work with families of diverse ethnic, cultural and socio-economic backgrounds. Ability to work flexible hours, including evenings and weekends as required. Valid driver’s license, current no-fault insurance coverage, satisfactory driver’s abstract, access to an insured vehicle. Continued employment in this position is contingent on successful completion of CPI classes OR other Behavioral Management certification as may be contract mandated. Continued employment in this position is contingent on successful completion of First Aid classes.
