Job Details

Provide onsite leadership and oversight of the hospital's Case Management department
Directly manage a team of 4 full-time Case Managers, 1 full-time Case Management Assistant, and 2 PRN Case Managers (mix of RNs and Social Workers)
Step in to personally cover patient caseloads as needed to support the team during high-volume periods or staffing gaps
Oversee utilization review and discharge planning processes across a 50-bed inpatient rehab unit with an average daily census of ~49 patients
Ensure timely, effective discharge planning given average patient length of stay of 7-14 days
Partner closely with the hospital CEO and Regional Director of Case Management on departmental strategy and performance
Ensure department compliance with regulatory, licensure, and clinical documentation standards
Support staff development, coaching, and performance management for Case Managers and Case Management Assistant
Pursue CCM or ACM certification within first year if not already obtained, with employer support
