As a Physical Therapist at Custom Home Health, you will:
- Deliver individualized care that empowers patients to achieve their health and independence goals.
- Collaborate with RNs and interdisciplinary teams to design and implement personalized care plans.
- Educate patients and their families on therapy techniques, exercises, and treatments to promote lasting well-being.
- Coordinate and administer treatments that enhance physical function and overall health.
- Approach every task with compassion, professionalism, and a commitment to excellence.
- Licensed Physical Therapist : Must hold a valid Physical Therapy license.
- 1+ Years of Physical Therapy Experience : Prior experience preferred, but exceptional new graduates are encouraged to apply.
