This Home Health Physical Therapist Assistant position offers the opportunity to care for patients in and around Lake Zurich, Illinois, a vibrant, welcoming village in southwestern Lake County. Lake Zurich is known for its scenic namesake lake, charming residential neighborhoods, and well-maintained parks and trails. Residents enjoy outdoor activities at Paulus Park and Breezewald Park, community festivals and events, and easy access to shopping and dining in the village center. With its highly regarded schools, safe environment, and convenient location roughly an hour from downtown Chicago, Lake Zurich combines the best of suburban comfort with access to big-city amenities. In this role, you will provide rehabilitative services to patients in their homes, helping them regain mobility, function, and independence following illness, injury, or surgery. Home health practice allows you to see firsthand how patients live and tailor interventions to the real-world environments they navigate every day. You will collaborate closely with a supervising Physical Therapist and an interdisciplinary team to implement individualized care plans and support patients in achieving their functional goals. A typical day may include traveling between patient homes within Lake Zurich and nearby communities, performing comprehensive assessments of mobility and safety, and delivering therapeutic interventions including strengthening exercises, gait training, balance work, transfer training, and instruction in use of assistive devices. You will also reinforce home exercise programs, provide education to patients and caregivers, and coordinate with the broader care team to address any changes in condition. Documentation will be completed using an electronic medical record system, with expectations for thorough, timely, and accurate visit notes. Patient caseloads in home health are generally managed to support one-on-one, focused care. You will often see a range of adult and geriatric patients, including those recovering from orthopedic surgeries, strokes, cardiac events, and other conditions impacting mobility. While patient ratios vary depending on scheduling and productivity standards, the emphasis in home health is on quality interactions where you can spend meaningful time with each individual. The position typically follows daytime hours with some flexibility based on patient needs and scheduling preferences. Weekend or holiday coverage may be occasionally required, depending on agency policies. You will have the opportunity to develop strong relationships with patients and families, exercise clinical judgment in diverse home environments, and build a robust skill set that is highly transferable to future roles in outpatient, acute care, or continued home health practice.Need Home Health PTA traveler for 13 weeks
