Housing Stability Case Manager

Socket

Homefull is seeking a Case Manager to coordinate all aspects of client care and case plans, helping clients obtain and maintain housing stability and independent living skills. You will work as part of a multi-disciplinary housing-focused team, conduct home visits, and document services in partnership with clients to improve outcomes. The role requires a Bachelor’s degree in related fields and a working knowledge of homelessness. #J-18808-Ljbffr
