Critical Access Hospital Position

This opportunity is with a facility that primarily serves outpatient needs (75%) alongside acute care and swing-beds (25%). The organization is committed to providing high-quality healthcare services and support to the surrounding community.

Role Responsibilities

• Deliver compassionate and competent care to patients in an outpatient and acute care setting.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Monitor and assess patient progress and adjust treatment plans as necessary.
• Maintain accurate patient records in accordance with regulatory standards.
• Participate in continuous improvement initiatives to enhance patient care quality.
• Educate patients and their families on health management and care options.
• Provide support and follow-up for patients transitioning to or from swing-bed and long-term care.

Qualifications

• Valid license and certification in the relevant healthcare profession.
• Experience in an outpatient or acute care setting preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Commitment to patient-centered care and continuous improvement.
