Requisition No: 874866 Agency: Agency for Persons with Disabilities Working Title: QSI Assessor – OPS – 67940135 Position: Senior Human Services Program Analyst – Other Personal Services (OPS) Position Number: 67940135 Competitive Opportunity: Open Competitive Telework: Eligible Salary: $22.00 hourly Position Summary This professional OPS position involves conducting evidence‑based assessments for the Agency for Persons with Disabilities using the Questionnaire for Situational Information (QSI). The role performs statewide productivity quality assurance procedures to ensure systematic monitoring and evaluation of programs and that standards of quality are met. Work You Will Do Complete assessments using the QSI, including reviewing records, conducting face‑to‑face interviews with the person, guardian, family, and relevant caregivers or service providers, verifying information, and entering results into the QSI online application. Review the person’s central record/iConnect record including recent assessments, progress notes, school records, previous support plans, and other collateral sources. Refer persons and family members to their Waiver or Pre‑Enrollment Support Coordinator for provider information and community resources based on needs identified during the QSI administration. Schedule meetings and follow‑up telephone sessions to conduct interviews with the person, family members, guardians, support coordinator, and providers to complete a comprehensive review of needs. Communicate a person’s needs for follow‑up by the Regional Medical Case Manager or Regional Behavior Analyst after completing the QSI. Verify that all demographic information is correct; if updates are required, alert the support coordinator to correct iConnect records. Attend meetings, complete training, and annually demonstrate inter‑rater reliability on the QSI for recertification as a QSI Assessor. Perform other duties as assigned, including administrative tasks. Minimum Qualifications Valid driver’s license or other efficient means of transportation for work travel. Bachelor’s degree and a minimum of four (4) years of professional experience working with persons with developmental disabilities. Excellent communication skills for person‑centered interviews and sufficient computer skills to use a web‑based assessment system and record results. Daily travel required to conduct assessments. Knowledge, Skills, And Abilities Knowledge of: QSI protocol, including scoring and interpretation. Person‑centered interview processes used in the QSI. Agency for Persons with Disabilities processes and procedures, including i‑Budget/Waiver and pre‑enrollment. Ability to: Use computer‑based systems such as iConnect, SharePoint, Microsoft Office, Microsoft Teams, and Excel. Communicate effectively in face‑to‑face, electronic, and written formats. Establish and maintain effective working relationships with persons, families, providers, and staff within the region and statewide. Prepare reports and manage, organize, analyze, and monitor QSI data. Administer approved functional needs assessments. Travel for work purposes.
