Registered Nurse - Home Health (PRN) Location: Farmington, MO, US Missions: Provide skilled nursing care to patients in their homes or assisted living facilities. Conduct individual assessments, develop care plans, and coordinate with physicians, nurses, therapists, and families to promote independence and recovery. Responsibilities Administer skilled nursing interventions and medications. Perform daily assessments and monitor patient progress. Communicate care updates to interdisciplinary teams and family members. Document patient care in electronic health records. Adhere to safety and infection control protocols. Support patients’ home‑based transition from hospital to recovery. Education & Credentials Graduate of an approved school of nursing (RN).
