Company: Oak Street Health Title: Medical Social Worker Location: 6232 Market St., Philadelphia, PA 19139 Role Overview The Medical Social Worker (MSW) is a key member of the Oak Street Health care team, providing assessments, advocacy, and comprehensive care coordination to highest‑risk patients and their caregivers. The MSW’s goal is to help patients improve health status, avoid hospitalizations, and live healthier and happier lives. Responsibilities Conduct comprehensive assessments, telephonic check‑ins, and in‑person visits for highest‑need patients. Partner with patients and support persons to identify goals, increase engagement, address barriers, and execute care plans. Respond to crises and urgent needs, triaging situations and multitasking as priorities shift. Maintain knowledge of community resources and provide appropriate referrals. Assist with navigating patient insurance benefits in collaboration with the Patient Relations Manager. Provide longitudinal and episodic care coordination, empowering patients to manage health conditions independently. Educate patients on supportive care options, including advance care planning and end‑of‑life care. Offer psychoeducation on life‑stage and disease‑specific issues (e.g., dementia, aging, caregiver support). Collaborate with care teams to develop creative solutions and intervene in complex psychosocial dynamics. Complete concise, comprehensive documentation in a timely manner. Partner with Community Health Workers (CHWs) to provide clinical guidance on complex patient needs. Perform other duties as assigned. Qualifications State of Pennsylvania licensure: Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) with a Master’s Degree in Social Work from an accredited program. Minimum one year of recent medical social work experience in a healthcare setting. Proficient computer skills (Microsoft, G‑Suite, electronic medical records). Experience with complex care coordination and/or population healthcare management. US work authorization. Strongly preferred: metric‑driven practice, positive attitude toward underserved populations, adaptability, multitasking, team collaboration, familiarity with local community resources, fluency in additional languages is a plus. Pay and Benefits Pay range: $51,256 – $111,351 per year (base salary; actual offer depends on experience, education, geography). Eligible for CVS Health bonus, commission, or short‑term incentive programs.
