If you are ready to be part of an extraordinary team of caregivers, then come work where you matter.Registered Dietitian Position OverviewThe Registered Dietitian is an experienced clinician who serves as a member of the clinical care team, meeting the needs of each patient based on their unique Plan of Care. In this role the Registered Dietitian plans, organizes and conducts dietetic education and supports patients and their caregivers as a member of the interdisciplinary team with expertise in nutritional care.Essential Functions and SkillsDevelops and implements a nutritional plan of care which is based on a comprehensive assessment of nutritional needs as part of the patient's total plan of care. Evaluates and reports results appropriately.The dietitian works with the referring physician and interdisciplinary group to provide appropriate counseling to individuals and families/caregivers in the home setting. They evaluate and instruct on nutritional principles, dietary plans, food selection and economics, and adapting plans to the individual's illness, as indicated. They will reevaluate nutritional care by providing home follow-up care either at the patient's residence or by phone.Communicates appropriate dietary history and nutritional care data through written record systems.Records subjective and objective information, assessment, treatment plan, and current progress notes in the patient's clinical records including a discharge summary.Attend interdisciplinary group meetings.Comply with the organization's policies and procedures. Maintains confidentiality and safety of patient records so that no information is divulged to other than proper users.Actively participates in quality assessment performance improvement teams and activities.Performs other duties as assigned.QualificationsRequirements/QualificationsWisconsin Registered Dietitian Credential.Master's degree in dietician studies from an accredited institution.Ability to pass background, consumer report, and drug test.Valid State Driver's License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.Physical RequirementsPushing/Pulling and Lifting/carrying up to 12 pounds. Occasionally up to 20 pounds.Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
