Registered Nurse Home Health

$1863.20/week -

Job Summary:

Provide comprehensive home health nursing care to patients by assessing needs, developing care plans, and delivering direct patient care in accordance with physicians orders and agency policies. Supervise and evaluate home health aides and licensed practical nurses/licensed vocational nurses to ensure quality care delivery. Coordinate visits, documentation, and patient management within the assigned territory.

Responsibilities:

• 
  Assess patient conditions and develop individualized care plans based on nursing process.
  
  
• 
  Administer medications, wound care, and other treatments as prescribed by physicians.
  
  
• 
  Supervise and evaluate home health aides and LPNs/VNs to maintain high care standards.
  
  
• 
  Document all patient visits accurately using HomeCare HomeBase (HCHB), ensuring completion within 24 hours.
  
  
• 
  Travel to patient homes within a 50-75 mile radius, ensuring timely and quality care.
  
  
• 
  Participate in scheduled team meetings and on-call rotations every 3-4 weeks.
  
  

Qualifications:

• 
  Hold a valid RN license in the state of practice.
  
  
• 
  Have at least 1 year of recent adult home health or hospice experience.
  
  
• 
  Proficient in Oasis documentation and experienced with HomeCare HomeBase (HCHB) EMR system within the last 6 months.
  
  
• 
  Possess reliable transportation, a current drivers license, and agency-required liability insurance.
  
  
• 
  Demonstrate excellent communication and documentation skills.
