Hours of Work :9-5Days Of Week :Monday-FridayWork Shift :8X5 Day (United States of America)Job Description :Job PurposeThe Care Transitions Navigator will coordinate activities that promote quality outcomes, patient throughput and discharge planning while supporting a balance of optimal care and appropriate resource utilization. The Care Transitions Navigator will identify potential barriers to patient throughput and quality outcomes minimizing delays in discharge plans.Job Requirements:Education:Bachelor's degree in Social Work requiredMaster's degree in Social Work preferredLicenses and/or Certifications:LMSW/LBSW, or RN as licensed by the Texas Board of Examiners, CCM or ACM preferredWork Experience:1 year of experience in health related settingHospital case management experience preferredRelated Work Experience and Other Skills:Ability to work independently, effective time management, problem-solving skills, detail-oriented with strong organizational skills, ability to multi-task, ability to react calmly, objectively, and effectively in emergency situations, maintain a high standard of ethical behavior and personal expertise in the performance of your work.Methodist Dallas Medical Center is one of North Texas’ best places to work.
