Availability- Start: ASAP upon credentialing
- Schedule: Monday-Friday, 8:00 AM-4:30 PM
- No call
- No weekends
- No after-hours
- No telehealth
- Ongoing outpatient coverage
- Availability to provide requested coverage dates required
- Active Illinois Medical License or IMLC
- Active Illinois DEA
- Active Illinois Controlled Substance Registration (CSR)
- Board Certified or Board Eligible in Internal Medicine or Surgery
- Wound Care & Hyperbaric Medicine training and experience required
- ACLS (AHA)
- BLS (AHA)
- PALS (AHA)
- Board Certified or Board Eligible in Internal Medicine or Surgery - Required
- Wound Care & Hyperbaric Medicine training and experience - Required
- Active Illinois Medical License - Required
- Active Illinois DEA/CSR - Required
- Availability for requested coverage - Required
- Two references per Stratum process - Required
- Clean, easy-to-credential candidates - Highly Preferred
- Outpatient Wound Care & Hyperbaric Medicine
- 100% Adult & Geriatric patient population
- 12-20 outpatient wound care visits per day
- 1-4 Hyperbaric Oxygen (HBO) treatments per day
- 8 exam rooms
- 2 Hyperbaric chambers
- Wound evaluations and treatment planning
- Compression therapy
- Wound VAC management
- Hyperbaric Oxygen (HBO) treatment oversight
- Debridements
- Cast applications
- Advanced tissue applications
- Incision and Drainage (I&D)
- Callous paring
- Chemical cautery
Optional Procedures
- Punch biopsies
- Experienced Wound Care RNs
- 1 Advanced Practice Provider in clinic Tuesday-Friday
- EMR: Epic
