Registered Nurse (RN) - Archbold Inpatient Hospice

Provide Hospice-specific nursing care to patients in homes, nursing homes, assisted living facilities or Hospice Inpatient Unit. Provide initial and ongoing counseling/education regarding illness, disease process, and/or bereavement support. Perform comprehensive Nursing Assessment on newly admitted patients. Evaluate data collection, formulate a plan of care and establish specific goals for the patient and family/support system. Work closely with physicians communicating pertinent observations. Regularly reassess the patient and update the care plan accordingly. Coordinate supplies and durable medical equipment, assuring delivery in a timely and cost-efficient manner. For inpatient and outpatient hospice care; coordinate inpatient, respite and acute admissions. For outpatient hospice care; coordinate outpatient services and treatments. Serves as patient/family advocate. Maintain patient/family dignity.

Experience/Qualifications: Current GA RN license. At least two years of experience, preferably in a hospice setting. For outpatient Hospice services, it may include extensive travel in the service area (must have clean driving record).
