Your Responsibilities

• Performing thorough patient assessments, recommending diagnostic tests, and creating individualized treatment plans
• Diagnosing and managing acute, episodic, or chronic conditions, either in conjunction with physicians or independently (as granted by your credentials)
• Providing education to patients and families on preventative health, wellness, and management of conditions
• Prescribing medications and recommending therapies aligned with your practice scope
• Engaging in patient follow-ups and chronic care strategies
• Collaborating with multidisciplinary teams to facilitate comprehensive, patient-centered care
• Accurate and thorough documentation utilizing electronic medical records (EMR)

Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Licensure and Certifications

• Active Nurse Practitioner license in Oregon
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or a relevant specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to acquire these within 90 days)
