Physical Therapist - Travel Contract

Physical Therapist - Home Health (PITTSFIELD, MA)

Provide skilled physical therapy to home health patients across age groups. Plan, implement, and evaluate individualized plans of care based on referrals and clinical assessments. Deliver therapeutic interventions for neuromuscular, neurological, and orthopedic conditions. Educate patients and caregivers on transfers, safe use of assistive devices, and home exercise programs. Collaborate with physicians and the interdisciplinary team. Document progress, revise plans of care, and maintain compliance with infection control and documentation standards. Travel locally and work flexible schedules as required.

Minimum Qualifications

• Current State Physical Therapist license valid in MA. Required.
• Bachelor of Science in Physical Therapy from an APTAapproved program or equivalent degree. Required.
• Proof of current CPR certification. Required.
• Current driver's license, reliable transportation, and auto insurance. Required.
• 1-2 years experience as a Physical Therapist in acute care or rehabilitation settings. Required.
• Experience with durable medical equipment and ordering/adjusting DME. Required.
• Ability to perform prolonged standing, walking, lifting, bending, pushing, and manual dexterity required for patient care. Required.
• Home care experience. Preferred.

Preferred Qualifications

• Previous home health experience. Preferred.
• Demonstrated competency in neuromuscular and orthopedic treatment techniques. Preferred.

Assignment Details

• Must be able to travel locally and float to other locations as needed.
• Ability to work a flexible schedule to meet patient needs, including potential evening or weekend visits.

Deliver patient-centered rehabilitation in the home environment to restore or maintain patient function and independence. Provide education to patients and families to support ongoing recovery and reduce hospital readmissions. Coordinate with interdisciplinary team members to ensure continuity of care and adherence to physician orders and regulatory requirements.
