This position is for a Physical Therapist (PT) in a Skilled Nursing Facility (SNF) working day shifts from 8:00 AM 4:30 PM, with 35 guaranteed hours per week for a 13-week contract. The PT provides rehabilitation services focused on improving patient mobility, strength, balance, and overall functional independence.

Requirements:

• Candidates must hold an active Physical Therapist license and have graduated from an accredited Physical Therapy program (DPT or PT).
• Experience in SNF, rehabilitation, or long-term care settings is preferred.
• Strong clinical assessment, treatment planning, and documentation skills are required.

Procedures:

• The PT will perform patient evaluations, develop individualized care plans, and deliver therapy interventions including gait training, strengthening exercises, balance training, and mobility improvement.

Responsibilities include monitoring patient progress, modifying treatment plans, documenting therapy sessions, and collaborating with interdisciplinary teams to ensure optimal recovery and safe discharge planning.
