Social Services Director

Job Description: The Social Service Director is responsible for identifying the medically related psychosocial needs of residents and providing services to meet those needs. The Social Service Director will address residents' intellectual, developmental, emotional, spiritual and social functioning so that each resident can attain or maintain the highest level of psycho-social functioning and well-being possible. Families will also be offered services in that their positive and continued involvement may be critical to the residents’ quality of life. The Belmont Health and Rehabilitation is seeking a compassionate Social Service Director to become a part of our team where our primary goal is to provide exceptional care for our residents. About the Role: The Belmont Health and Rehabilitation is seeking a Social Services Director to- Assist in the entire admissions process from pre-admissions (pre-screening, tours, etc.) to discharge (placement arrangements, homecare referrals, etc.) Provide social services in accordance with accepted facility, professional and governmental regulatory standards. Act as a resident advocate and liaison with facility staff and with community resources as necessary.
