Position Summary In House Rehab Team Responsibilities Provides physical therapy services to residents under the direct supervision of a physical therapist. Identifies indications for and assists in evaluations at appropriate intervals under the direction of the physical therapist. Assists the physical therapist in identifying a problem list, short-term goals, long-term goals and treatment plan. Maintains and completes accurate documentation, department records and reports.
