Home Health Occupational Therapist

Location: Medford, OR and Surrounding Areas

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift, 5x8-hour (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Job Description: We are seeking a dedicated Home Health Occupational Therapist to provide exceptional care to patients in their homes. The therapist will deliver therapy services to improve clients' functional abilities and enhance their quality of life.

Responsibilities:

• Conduct evaluations and develop treatment plans tailored to individual patient needs.
• Implement therapeutic interventions to assist patients in achieving their rehabilitation goals.
• Document patient progress and modify treatment plans as necessary.
• Coordinate care with other healthcare professionals when required.
• Educate patients and families on home exercise programs and safety measures.

Required Experience / Certifications / Licensure:

• Minimum of 1 year of skilled home health experience required.
• Current BCLS (Basic Cardiac Life Support) certification.
• Experience with Oasis documentation is required.
• Must have reliable transportation and be willing to drive to multiple locations, with some visits up to 50+ miles.
• COVID-19 vaccination (primary series plus one booster) is required.
