Our model supports meaningful visits, thoughtful care planning, and true connection with patients and families — not rushed encounters.What You Will Do as a Hospice RN Case Manager:Serve as a key member of the Interdisciplinary Team (IDT), providing holistic, patient- and family-centered hospice careComplete comprehensive nursing assessments, admissions, and ongoing reassessments in accordance with hospice regulations and timelinesDevelop, implement, and continuously evaluate individualized plans of care in collaboration with the IDT and attending physiciansProvide skilled nursing care, including symptom management, pain control, wound care, IV therapy, and physical assessmentsEducate patients and families on disease progression, medications, pain and symptom management, and routine caregiving tasksProvide emotional support and compassionate guidance to patients and families throughout the hospice journeyFulfill on-call responsibilities as assigned and assist with admissions or coverage during staffing needsWhat We Are Looking ForCurrently licensed as a Registered Nurse in the Commonwealth of VirginiaMinimum of two (2) years of nursing experienceHospice or home health experience preferredStrong clinical judgment, critical thinking, and decision-making skillsExcellent communication skills with the ability to educate, support, and collaborate effectivelyProficiency with EMR documentation and computer-based systemsValid driver's license, reliable transportation, and proof of automobile insuranceSchedule & BenefitsSchedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM–5:00 PM)Location: New River Valley Service AreaBenefits: 21 days of PTO in your first year, $.65 per mile mileage reimbursement, company paid life insurance and long-term disability insurance, medical through an ICHRA model offering multiple plan choices, and 401(k) with company match (100% match on the first 3%, 50% match on the next 1%)Starting Salary: 75,000/yearThis is the starting salary for the position.
