Senior Case Manager — Housing & Community Rehab

ACMH is seeking a Senior Case Manager/Case Manager for the Bronx Treatment Apartment (BTA). The role provides case coordination and delivers community rehabilitation services in The Bronx, with a Monday–Friday, 9:00am–5:00pm schedule. Reports to the Program Director and collaborates with the treatment team to help clients access psychiatric, medical, and entitlement services.
