Work Environment

This role takes place onsite at our Jacksonville rehabilitation facility, where you will collaborate with a team of experienced healthcare professionals in a supportive and dynamic atmosphere. The facility is designed to foster collaboration and innovation, enabling practitioners to provide comprehensive therapy that meets the diverse needs of our patients. Working Monday through Friday in a standard 40-hour shift, you will experience a consistent and structured schedule that supports work-life balance.

Key Responsibilities

  • Conduct thorough assessments to evaluate patients' conditions, creating individualized treatment plans based on physician referrals.
  • Implement diverse treatment modalities aimed at enhancing mobility, function, and overall quality of life.
  • Maintain effective communication and rapport with patients, demonstrating exceptional customer service throughout their rehabilitation journey.
  • Collaborate with multidisciplinary teams to ensure cohesive and evidence-based care for optimal patient outcomes.
  • Administer specialized therapeutic techniques both in-clinic and, when necessary, provide care at patients' homes to adhere to the physician’s plan of care.
  • Educate patients and caregivers on treatment protocols, exercises, and lifestyle modifications to enhance recovery.
  • Meet and exceed established productivity and quality metrics to maintain high standards within the practice.

Patient Population / Client Focus

Our patient population is diverse, encompassing individuals recovering from orthopedic injuries, pediatric developmental challenges, neurologic conditions, and sports-related traumas. As a Physical Therapist at Convivial, you will encounter a varied caseload that allows you to apply your expertise across specialties. Each patient is treated with a personalized approach that emphasizes compassion and understanding, ensuring that their unique needs are met.

Qualifications

  • A Doctorate in Physical Therapy (DPT) from an accredited program is required.
  • Active state licensure as a Doctor of Physical Therapy (DPT) through the Physical Therapy Examining Committee.
  • A minimum of two years of clinical experience is necessary, giving you hands-on exposure to a wide range of therapeutic practices.
  • Certifications in CPR and emergency first aid are required to ensure the safety of our patients and staff.
  • Exceptional written and verbal communication skills are essential for effective collaboration with team members and education of patients.
  • Proficiency in using digital tools for documentation and treatment planning enhances workflow efficiency and ensures comprehensive patient care.

Preferred Experience

  • Experience with evidence-based practice guidelines and the ability to integrate research into clinical decision-making.
  • Familiarity with EMR (Electronic Medical Records) systems to maintain precise patient documentation and treatment histories.
  • Background in working with community outreach programs or initiatives that promote health and wellness.

Compensation and Benefits

Compensation for this position ranges from USD 100,000 to 120,000 per year, commensurate with experience and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package that includes:
