Home Health Physical Therapist (Contract)

Platinum Medical Staffing is seeking licensed Home Health Physical Therapists to join our team under a continuous contract to provide in-home therapy services throughout Northern Virginia. As a contracted clinician, you'll manage a consistent caseload, delivering high-quality care to patients in the comfort of their own homes.

Key Responsibilities:

• Deliver in-home physical therapy in accordance with physician orders
• Complete SOC, ROC, evaluations, follow-up visits, and recertifications as required
• Conduct comprehensive fall risk and functional mobility assessments
• Create and implement individualized Plans of Care (POC)
• Document all care accurately in Homecare Homebase (HCHB) within 24 hours
• Maintain regular communication with skilled nursing and care coordination teams
• Confirm appointments with patients and promptly report any schedule updates

Requirements:

• Active Physical Therapist license in Virginia
• Valid CPR certification
• Familiarity with OASIS-E documentation standards
• Experience using Homecare Homebase (HCHB) preferred, or willingness to learn
• Excellent time management and communication skills
• Reliable transportation and willingness to travel locally
