Deliver impactful patient care in a dynamic home health environment as a Nurse Practitioner conducting risk assessments throughout Santa Fe County, NM. In this per diem role, youll have meaningful, face-to-face interactions with Medicare Advantage members in their homes, supporting both their ongoing wellness and transitional care needs.

Schedule/Hours:

• Flexible shifts available Monday through Sunday

• Minimum commitment required weekly or bi-weekly

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage members
• Collect and evaluate detailed medical histories, conditions, and medication information
• Provide clear education related to health conditions and prescribed medications
• Assess for potential care gaps, communicating actionable recommendations for improvement
• Visits may include annual health assessments or post-discharge evaluations; each typically takes about 45 minutes
• No treatment planning or prescribing necessary

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Board Certification (AANP or ANCC)
• MSN degree from an accredited program
• Current CPR certification (BLS)
• At least 6 months of recent NP experience preferred

What Youll Receive:
