Description

• Conduct patient assessments in both inpatient and outpatient settings, developing tailored treatment plans
• Implement therapeutic interventions emphasizing mobility, strength, balance, and overall function
• Engage collaboratively with nurses, physicians, case managers, and rehabilitation professionals to enhance patient outcomes
• Inform patients and their families about therapeutic goals, discharge logistics, and at-home exercise programs
• Maintain detailed documentation of evaluations, treatments, and patient progress according to facility standards
• Participate in care team meetings and interdisciplinary discussions to ensure comprehensive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Must hold state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., in neurology or geriatrics) are beneficial but not mandatory
