Field Outreach Specialist — Community Health Connector
Pacific Health Group
Pacific Health Group is seeking a Street Outreach Advisor to connect individuals with care in California. This hybrid, field-based role focuses on building partnerships, conducting in-person outreach, and expanding access to Enhanced Care Management, Community Supports, and Behavioral Health services. You will spend most time in the community, travel within the county up to 60-70%, document activities in CRM, and collaborate with internal teams to ensure referrals are tracked and closed. #J-18808-Ljbffr Pacific Health Group
