Overview ChenMed is a physician‑led, primary care organization focused on serving seniors through Medicare Advantage. We are expanding rapidly and seek dedicated Primary Care Physicians (PCPs) who share our mission of ensuring affordable VIP care, honoring seniors, and improving health outcomes. Key Responsibilities Accountability for outcomes: Provide high‑quality, cost‑effective clinical care to a panel of up to 450 patients, build trusting relationships, and influence patient behavior to improve outcomes. Health coaching: Act as a health coach, helping patients set short‑ and long‑term goals, partnering to achieve them, and following up to support progress. Simplify and prioritize care: Translate care plans into actionable steps for patients and lead care teams to meet performance goals. Deliver care for acute and chronic illnesses in older adults, including assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Collaborate with hospitalists, transitional‑care teams, case managers, and other specialists; engage hospitalists regardless of affiliation. Lead a care team consisting of a medical assistant, care facilitator, and care coordinator; cover for other providers as needed. Assist in recruiting patients and additional providers for the center and company. Maintain compliance with guidelines, risk and quality‑management programs, clinical meetings, and company policies. Performance Metrics Key metrics include patient admissions per thousand, appointment slot utilization (18‑21 slots per day), CGCAHPS scores, clinical gap closures, and medical cost measures. PCPs have individualized goals and work toward them with center and market leadership. Knowledge, Skills & Abilities Availability: Provide walk‑in visits, answer calls and messages promptly, and be accessible to patients. Service orientation: Deliver care you would expect for a family member. Evidence‑based medicine: Stay current on clinical, nutritional, and lifestyle interventions. Physician leadership: Continuously develop leadership skills for patients, teams, centers, and the organization. Quality focus: Work with teams to achieve high‑quality outcomes and improve practice operations. Influence: Compassionately influence patients and teams to achieve the best outcomes. Self‑care: Maintain personal health and wellbeing to provide optimal care. Technical proficiency: Use Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and related software. Travel: Willingness to travel locally, nationally for initial training, and regionally for recruiting or training. Language: Fluency in English (verbal and written); other languages may be required for specific centers. Education & Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Active State Medical License in the state(s) of practice. Board certification in the specialty preferred; board eligibility required; maintain certification through MOC/CME. Current DEA number for schedule II‑V controlled substances.
