Health System Specialist

The Health Systems Specialist 2 (Social Work) works with an interdisciplinary team in the Center for Hospice and Palliative Care at the NYS Department of Health in Syracuse to integrate research, policy, and training around patient-centered care. The role focuses on discovering deficiencies, improving communication, and increasing education on hospice and palliative care, with expertise in advance care planning, bereavement support, and care coordination. The incumbent will develop quality assurance and process improvement workplanes, assist with website updates, and coordinate education and outreach campaigns. Key qualifications: Licensed Master Social Worker (LMSW) with New York State license and current registration, OR Licensed Clinical Social Worker (LCSW) One year of post-licensure experience providing clinical services in a licensed healthcare facility or utilizing clinical knowledge in clinical services review and oversight Strong time-management, critical-thinking, and problem-solving skills Effective communication with internal and external stakeholders and strong MS Office skills Preferred: experience in Hospice, Palliative Care, Grief Counselling, or Advance Care Planning #J-18808-Ljbffr
