Nurse Case Manager

SUMMARY: Reports to the Manager or Director of Case Management and provides coordinated care support to facilitate patient care services. Participates in daily rounds and collaborates with the clinical healthcare team across the patient care continuum, including pre‑admission and post‑hospital discharge. The role helps implement discharge plans and ensures efficient delivery of patient care through appropriate utilization of healthcare resources. Full‑time, part‑time, per diem and job‑share schedules available. RESPONSIBILITIES Partner with medical staff and other healthcare team members to facilitate the plan of care for a defined patient population across the continuum of care. Identify high‑risk patient populations within the caseload for care‑management assessment, screening, and targeted interventions within one business day of admission. Participate in daily care rounds to evaluate and facilitate development and implementation of the discharge planning process. Develop the initial patient discharge plan, review it with patients, families, and the interdisciplinary team, and reassess the plan daily during collaborative rounds. Build post‑hospital referrals and send them to the Transition Care Coordinator when indicated to facilitate timely discharge. Deliver important message follow‑up notices to all Medicare patients according to CMS regulations. Follow CMS and DOH regulations regarding discharge guidelines and patient rights. Coordinate length of stay with the physician care team and patients; ensure the team is informed of insurance qualifiers that may affect the discharge plan. Discuss discharge readiness with patients and identify and assess readmitted or complex patients in collaboration with the healthcare team. Advocate for patients and advise on financial implications of discharge plans. Communicate the discharge plan, including post‑facility agency acceptance, to patients, families, and all care team members. Document final discharge disposition in progress notes. Develop appropriate patient care reports to ensure safe handovers when a patient is transferred from one care area to another. Provide care‑plan direction to support a patient‑care delivery system that reduces readmission risk. Act as a change agent by identifying opportunities to improve patient flow and reduce service delays through problem resolution and follow‑up. Participate in departmental and interdepartmental quality improvement activities, such as OpX weekly LOS reviews, interdisciplinary rounds, and readmission reviews. Participate in orientation of new staff and ongoing professional education to maintain an advanced level of knowledge in patient care services, third‑party payer, and managed‑care requirements. QUALIFICATIONS Registered Nurse license in the State of Rhode Island or by the National Council of State Boards of Nursing Compact. Graduate of a School of Nursing with a current R.N. license in Rhode Island. Bachelor’s degree required; certification in case management by a nationally recognized organization preferred. American Heart Association Basic Life Support (BLS) provider required. Three years of clinical experience, preferably with recent experience in case management, community case management, patient navigation, or discharge planning. Strong analytical and interpersonal skills for communicating daily with healthcare professionals, patients, and families. Proficiency in Microsoft Office, including email, Outlook calendar, and basic keyboarding. Knowledge and skills necessary to provide care across the lifespan with sensitivity to aging processes, human development stages, and cultural patterns. WORK ENVIRONMENT & PHYSICAL REQUIREMENTS General hospital environment with occasional stressful conditions associated with patient care. Adherence to Hospital Infection Control policy to minimize exposure to blood‑borne pathogens. Ability to walk through various patient‑care areas or use mobile means for rounds. Visual acuity and finger dexterity for reviewing and carrying medical records and using computer systems. Ability to lift up to 10 lbs. when transporting items between units. BENEFITS & ADVERTISEMENT Pay range: $90,417.60 – $146,640.00. Location: Rhode Island Hospital, 593 Eddy Street, Providence, Rhode Island 02903.
