Poway Adult Day Health Care Center provides professional care in a social environment to older adults with chronic health challenges. We utilize a multidisciplinary team approach (nursing, social services, physical therapy, occupational therapy, therapeutic activities, etc). We specialize in memory care.

Social Worker job duties include:

Assist in providing social work services for Poway Adult Day Health Care Center including:

• Facilitate access to support services provided at the center by coordinating/overseeing client schedules and transportation to and from the program daily and weekly.
• Assist with behavior management on the main activity floor.
• Answer incoming phone calls on main line and provide tours to potential clients.
• Conduct social work assessments and treatment planning.
• Provide counseling to individuals and families.
• Provide case management including information/resource coordination.
• Responsible for daily, weekly, monthly, and quarterly clinical notes and record keeping.
• Conduct home visits as needed.
• Assist with additional clerical tasks as needed.
• Mild physical tasks required as needed (assist participants to and from vehicles, assist participants up from chairs, serving lunch)

The social worker hired will be working directly on a team with social work assistants providing all required social work and related tasks.

Minimum Qualifications (Requirements are from state Title 22 regulations, and applicants who do not meet the following requirements will be rejected for the position):

“Social worker” shall be a person who meets one of the following:

• (1) The person holds a master’s degree in social work from an accredited school of social work. OR
• (2) The person holds a master’s degree in psychology, gerontology, or counseling from an accredited school and has one year of experience providing social services in one or more of the fields of aging, health, or long-term care services. OR
• (3) The person is licensed by the California Board of Behavioral Sciences . OR
• (4) The person holds a bachelor’s degree in social work from an accredited school with two years of experience providing social services in one or more of the fields of aging, health, or long-term care services.

Schedule is Full Time: Mon-Fri 8am-4:30pm

No weekends required.
