Care Coordinator

$20 - $25 per hour

If you enjoy helping others, then this is the place to start your career. Community Options has an opening in our Behavioral Health Department for a Care Coordinator. This role involves managing client caseloads within assigned service programs. You will build strong client relationships, coordinate support with other service providers, and make referrals to internal and external resources. Additionally, you will facilitate client team meetings and ensure follow‑up to achieve client goals. Responsibilities Conduct client intakes, assess needs, and develop care plans to promote client independence. Build and maintain strong client relationships. Qualifications Associate degree in a related field required, OR 1 year of experience in a direct service setting (e.g., DSP, CNA, human services, customer service). Valid driver’s license, acceptable driving record, reliable transportation, and proof of insurance. Ability to travel, including to outer or surrounding regions as assigned. Successfully complete care coordination training provided by the State of North Dakota after being hired. Pass a criminal background check, drug screening, and obtain ND Department of Human Service pre‑employment approval. Complete training as required by Community Options. Understanding of human services and developmental disabilities. Demonstrated ability to lead people and achieve results through others. Team building skills and ability to plan over a one‑to‑two‑year time span. Ability to manage resources and multiple projects while meeting regulatory and program requirements. Problem‑solving skills: develop solutions at a functional and strategic level. Knowledge of company policies and procedures; basic accounting skills. Strong customer service orientation and commitment to company mission and values. Make ethical and professional decisions. Accurate observation and recall of surroundings and events. Clear and concise written and verbal communication in English. Basic computer‑related task competence. Organize and manage tasks, provide quality customer service, and work as part of a team. Physical Demands and Work Environment The duties for this position require climbing stairs, balancing, stooping, kneeling, crouching, reaching above shoulder height, bending, twisting, sitting, standing, walking, pushing, pulling, lifting, grasping, fine motor manipulation, talking, hearing, and repeating motions. Occasionally, you may lift or move objects up to 50 pounds. Specific vision abilities required include close, distance, color, peripheral, and depth perception. Employees may encounter customers that exhibit de‑escalating behaviors and may need to use physical de‑escalation techniques. Work occurs in indoor and/or outdoor settings, including office, home, or community contexts, and requires operating a motor vehicle and traveling as needed.
