We are looking for a Physical Therapist who believes meaningful rehabilitation happens beyond clinic walls—in the homes and communities where life truly happens. In this role, you’ll work with individuals with neurological diagnoses to rebuild strength, improve balance, and restore safe mobility in the environments they navigate every day. Your care will help patients regain independence with daily activities, prevent falls, and build the confidence needed to move through life safely. Driving, travel, mileage reimbursement. Educational programs. Growth/advancement opportunities. Responsibilities What you will do: Perform evaluations and develop treatment plans. Regularly re‑assess effectiveness of treatment plans, attend staffing meetings and family conferences. Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources. Communicate patient’s needs and progress to the treatment team, physician, service recipient, and family members. Communicate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes. Qualifications What you will need: Minimum of bachelor's degree in Physical Therapy from an accredited Physical Therapy program. Current, unrestricted license as a Physical Therapist in the state of practice. Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations. Minimum of one year’s work experience as a physical therapist preferred. Knowledge of rehabilitation techniques related to complex neurological injury. Effective and professional communication in verbal and written interactions. Ability to lift 50 pounds. Capability to move, lift, or transfer patients, potentially lifting up to 100 pounds following safety procedures. Duties require fine motor skills, visual acuity, and walking/standing for extended periods. Additional physical requirements: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently. Health screening or examination may be required prior to assignment and periodically thereafter. Pay USD $50.00–$60.00 per hour. #J-18808-Ljbffr
