Bachelor’s degree in psychology, Social Work, Human Services, or a related field Two to three years of relevant community-based experience in behavioral health, care management, case management, or social services Knowledge of behavioral health, chronic health conditions, community resources, and care coordination. Harmonia Collaborative Care is seeking a full-time Health Homes Case Manager to support adults in Erie County who are navigating behavioral health needs, chronic medical conditions, and barriers to care. This role is for someone who understands that progress is often built one connection at a time, a provider appointment, stable housing, transportation, benefits, food access, or simply having someone who follows through. Responsibilities Manage a caseload of approximately 45–50 adults enrolled in Medicaid Health Home services Complete intake, screening, eligibility reviews, assessments, and reassessments Identify client strengths, needs, barriers to care, service gaps, and available supports Develop individualized care plans with clear short- and long-term goals Coordinate care with behavioral health providers, primary care, specialists, and community agencies Connect clients with housing, benefits, food resources, legal supports, financial assistance, and other community services Monitor progress, support transitions of care, and advocate for client-defined outcomes Maintain accurate, timely electronic case records in accordance with NYS Health Homes and Medicaid standards Schedule and Setting Full-time position Based in Derby, with home visits and community travel throughout Erie County Primarily office-based with some work-from-home flexibility Some evening hours may be required No weekends Join Harmonia and help make care more connected, accessible, and personal. #J-18808-Ljbffr
