Home Health RN Case Manager - Day Shift, Sign-On Bonus

$31 - $46.25 per hour

Position Details Location: Avera Health Broadband Lane Worker Type: Regular Work Shift: Day Shift (United States of America) Pay Range: $31.00 - $46.25 (actual pay rate dependent upon experience) Position Highlights Monday‑Friday, 8:00 am‑4:30 pm; 72 hrs per 2 weeks Weekend rotation: 1 weekend every 7 weeks Holiday rotation: 1 holiday every 2 years Call time involvement: 4‑5 call shifts per 7‑week schedule Compensation May be eligible for a $15,000 sign‑on bonus. Student Loan Repayment: each month an extra payment of $167 (up to $10,000) to help pay down your loan principal faster. Overview Responsible for care planning, utilization of services, coordination of care, and education of interdisciplinary teams in the management of patient care services. Develops all patient care plans in collaboration with the physician and interdisciplinary team, providing patient‑centered care efficiently, effectively, and with the best possible outcomes. Responsibilities Function as an interventionalist at home using clinical expertise and problem solving to make recommendations to physicians based on comprehensive assessments. Implement the nursing process using proficient assessment skills during start‑of‑care visits, post‑hospital evaluations, patient re‑certifications, discharges, and supervisory visits. Partner with physicians and the interdisciplinary team to develop the plan of care in collaboration with the patient. Make recommendations for efficient and effective utilization of resources during admission visits as dictated by the patient’s diagnosis. Initiate and respond to requests for changes in plans of care, determine appropriateness of change, and assess whether a change would be considered a Significant Change in Condition requiring an OASIS. Demonstrate the ability to influence others to follow recommended alternatives to care, providing thorough instruction and disease‑management education to patients and caregivers as ordered. Educate patients, families, and caregivers about disease processes, community resources, and recommended self‑care. Qualifications Registered Nurse (RN) – Board of Nursing license required. 1–3 years of experience in home health or hospice care preferred. Must be able to perform all essential job functions, including adequate visual acuity, effective communication, hearing, and sound discrimination.
