Physical Therapist (PT) – Full-Time & Per Diem (PRN) In-House Subacute Rehabilitation Nexus at Berwyn 3601 S Harlem Avenue, Berwyn, IL 60402 Job Type: Full-Time & Per Diem (PRN) Setting: In-House Subacute Rehabilitation & Skill Nursing Job Summary Nexus at Berwyn is seeking compassionate and motivated Physical Therapists (PTs) to join our in-house rehabilitation team in Berwyn, Illinois. We are hiring for both Full-Time and Per Diem (PRN) positions in a subacute rehabilitation and skilled nursing setting. The ideal candidate will evaluate and treat residents with physical impairments, functional limitations, and mobility-related conditions while working collaboratively with an interdisciplinary team focused on improving patient outcomes, independence, and quality of life. Responsibilities Evaluate, diagnose, and develop individualized treatment plans for residents with physical impairments and functional limitations Provide skilled physical therapy services in accordance with physician orders and facility policies Implement therapeutic exercises, gait training, transfer training, balance activities, and functional mobility interventions Perform ongoing reassessments and adjust treatment plans based on patient progress Maintain accurate and timely documentation in compliance with state, federal, and facility guidelines Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families regarding patient progress and discharge planning Educate patients, caregivers, and staff on mobility techniques, safety awareness, and fall prevention strategies Participate in care plan meetings and interdisciplinary team discussions Ensure compliance with all regulatory and ethical standards Qualifications Doctorate or Master’s Degree in Physical Therapy from an accredited program Current Illinois Physical Therapist license required Experience in skilled nursing, subacute rehabilitation, or geriatric settings preferred Strong clinical reasoning, communication, and interpersonal skills Ability to work independently and as part of a collaborative team Benefits Full-Time Position Competitive compensation Health, dental, and vision insurance Paid time off Continuing education opportunities Supportive in-house rehabilitation team Career growth opportunities Per Diem (PRN) Position Flexible scheduling based on facility needs Competitive hourly compensation Opportunity to work in a supportive rehabilitation environment Collaboration with an interdisciplinary care team Why Join Us?
