RN Case Manager - Hospice

Job Title: RN Case Manager – Hospice Location: Atlanta, GA Metro Area (Northwest and Northeast of the city: Roswell, Marietta, Alpharetta, Canton, Johns Creek, Kennesaw, etc) Position Type: Full-Time Position Overview We are seeking a compassionate and experienced RN Case Manager to join our hospice team. In this role, you will manage a caseload of patients receiving end-of-life care, ensuring comfort, dignity, and quality of life through individualized, patient-centered care plans. The ideal candidate is a strong clinician with hospice experience who is comfortable supporting patients and families through the end-of-life process and collaborating closely with an interdisciplinary care team. Key Responsibilities Case Management & Care Planning Manage a defined caseload of hospice patients across home and/or facility settings Conduct comprehensive assessments and ongoing reassessments Develop and implement individualized hospice care plans in collaboration with the interdisciplinary team Coordinate all aspects of patient care to ensure continuity and quality Clinical Care Provide skilled nursing care, including pain and symptom management Administer medications and treatments as ordered Monitor patient condition and respond to changes appropriately Ensure care aligns with hospice philosophy and physician orders Patient & Family Support Serve as primary point of contact for patients and families Provide education and emotional support related to disease progression and end-of-life care Guide families through the hospice process and care decisions Participate in on-call rotation as required Interdisciplinary Collaboration Work closely with physicians, social workers, chaplains, and hospice aides Participate in interdisciplinary team (IDT) meetings Communicate changes in patient status and care needs Documentation & Compliance Maintain accurate, timely, and compliant clinical documentation Ensure adherence to all Medicare hospice Conditions of Participation and regulatory requirements Participate in audits, surveys, and quality improvement initiatives Qualifications Active Registered Nurse (RN) license in Georgia 1–2+ years of nursing experience required; hospice or home health experience strongly preferred Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Compassionate, patient-centered approach to care Ability to work independently and manage a caseload effectively Compensation & Benefits Competitive salary based on experience Mileage reimbursement Health, dental, and vision insurance Paid time off and holidays Supportive team environment What Success Looks Like Effective pain and symptom management for all patients High patient and family satisfaction Timely and accurate documentation Strong collaboration with interdisciplinary team Apply Today If you are a compassionate RN committed to providing high-quality end-of-life care, we encourage you to apply and make a meaningful difference in the lives of patients and families. #J-18808-Ljbffr
