LPN (Licensed Practical Nurse)

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility team. The ideal candidate will provide high-quality nursing care to elderly patients, including those with dementia and other behavioral health conditions.

Key Responsibilities

• Deliver patient-centered nursing care to residents, ensuring their physical and emotional well-being.
• Collaborate with nursing staff and healthcare professionals to provide comprehensive care plans.
• Administer medications and treatments as prescribed by the medical team.
• Maintain accurate patient records and documentation using electronic medical records (EMR).
• Supervise and support medication aides during shifts.
• Participate in ongoing staff training and orientation programs.

Shift Details

• 12-hour day shifts with a requirement to work every other weekend and holidays.
• Guaranteed hours: 36 hours per week.

Qualifications

• Current LPN license in the state of practice.
• Basic Life Support (BLS) certification is required.
• A minimum of 1 year of nursing experience, preferably in a long-term care or skilled nursing environment.
• Strong interpersonal and communication skills, with the ability to work effectively in a team.
• Experience with electronic medical records systems, such as PCC, is preferable.

Patient Demographics

The facility cares for an elderly population, including individuals with dementia and behavioral health issues such as schizophrenia, bipolar disorder, and paranoia. Many patients manage their conditions effectively with appropriate treatment plans.

Additional Notes

• Orientation will include training and competencies, as well as the opportunity to shadow experienced staff for 1-2 shifts.
• There will be a med aide present from 6:00 AM to 10:00 PM and 1 nurse available from 10:00 PM to 6:00 AM for support.
