This position involves working within various counties in Wisconsin, performing home visits and collaborating with care teams to provide comprehensive patient care. Candidates will work in home and healthcare settings, aiding patients across Fond du Lac, Dodge, Columbia, and Winnebago counties.

• Perform comprehensive assessment of high-risk members

Requirements of the RN - Case Management - Travel

• RN License for WI
• Wound Care Experience
• Certified CPR/BLS
• Valid Driver's License
• Reliable Vehicle with Valid Car Insurance
• Associate's or Bachelor's Degree in Nursing or related field
• Experience with Electronic Medical Records; Home Care Home Base highly desired
• Case Management certification
