About the Role
This pivotal, field-based role focuses on building and managing strong referral relationships with physicians, hospital case managers, skilled nursing facilities, and assisted living communities. The ideal candidate is both relationship-oriented and results-driven, with a track record of generating measurable growth in home health or healthcare business development.

Key Responsibilities

• Develop and execute strategies to increase referrals and patient census
• Build, maintain, and grow relationships with physicians, case managers, and healthcare partners
• Identify and secure new referral sources through consistent outreach
• Represent Luxe Home Health professionally in the field and community
• Track referral activity, market trends, and competitor activity to inform strategy
• Maintain working knowledge of Medicare home health regulations to ensure compliance
• Collaborate with clinical and operations teams to ensure smooth patient transitions and strong customer experience

What We’re Looking For

• 2+ years of experience in home health, hospice, or healthcare business development/marketing
• Proven ability to generate and grow referral volume within a defined territory
• Strong communication, relationship-building, and follow-up skills
• Self-motivated with the ability to work independently
• Healthcare knowledge preferred, including Home Health Medicare regulations
• Valid Illinois driver’s license and reliable transportation
