Hospice RN Field Nurse Home-Based Care
Communicare Health Services
CommuniCare Health invites applications for a Hospice RN Field Nurse. This role plans, organizes and directs home care services with emphasis on community health education, delivering patient-centered care in the home environment.
You will assess patients, supervise LPNs and aides, develop care plans, and administer medications as prescribed while coordinating discharge planning and acting as Case Manager for an assigned caseload.
