Thispatient-facingrole is essential in helping our most vulnerable community members overcome barriers to care and improve their overall wellbeing.

*This position will also cross train as an Intake Coordinator to aid as back up when there is short staffing.*

Type:Full-Time (40 hrs/week) - Monday through Friday 8am-5pm

Key Responsibilities:

• Coordinate Care:Facilitate warm hand-offs and referrals between patients, providers, and community services.
• Screen & Support:Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers.
• Educate & Advocate:Assist with applications for coverage, benefits, and other essential services.
• Document Thoroughly:Maintain detailed electronic patient records, tracking outcomes and service plans.
• Engage the Community:Participate in outreach events and consortiums to promote One Health services.
• Navigate Resources:Stay current on eligibility rules, insurance options, and enrollment procedures.

What Were Looking For:

• Strong computer and documentation skills
• Ability to manage multiple priorities with remote supervision
• Professionalism, empathy, and clear boundaries
• Community involvement or connection to the population served
• High School Diploma or GED
• Valid Drivers License

Certifications (upon hire or within first year):

• BLS Certificate
• Community Health Worker Certificate
• Certified Application Counselor (CAC)
